Palliative care has been shown to improve quality of life by addressing pain and reducing stress. In some circumstances even improve survival for those facing a terminal illness.

Supporting Patients Through Serious Illness

Palliative care treats symptoms and relieves suffering for anyone with a serious illness – be it life-threatening, chronic or curable. We help improve the quality of life for you and your family at any stage in the disease process.

Our team of palliative specialists – physician, nurse practitioner, social worker and others – meet with you and your family to set treatment goals and to make sure treatment options honor your wishes and concerns.

We Offer Inpatient and Outpatient Care

Outpatient care is offered at our practice — Foundation Palliative & Supportive Care, which includes evaluations, follow-up visits and other care as needed provided through the Palliative Care team. Inpatient-care is offered for patients who are admitted at Southern New Hampshire Medical Center.

Ask your primary care provider or medical team for palliative support early in the healing and recovery process. If you have a serious illness, you can ask for palliative care at any time.

Working together with your primary care provider, our palliative specialists can help:

  • Treat complex pain and symptoms
  • Manage related issues such as anxiety, depression, nausea, loss of appetite and fatigue
  • Address caregiver stress and fears
  • Smooth the transition from one care setting to another (for example, from the hospital to home)
  • Provide guidance and support to you, your family and your medical team
  • ​Lead family discussions to be sure everyone agrees about treatment goals and approaches and help settle any misunderstandings regarding treatment
  • Explain choices at the end of life to improve your overall comfort and quality of living
