Nurse practitioners (DNP, NP, PNP, APRN, FNP) work independently, serving as primary care providers.

Our primary care providers work collaboratively with our specialists to ensure that you receive integrated, coordinated and timely care. Through shared Electronic Medical Records, all of our providers at Foundation Medical Partners have quick and easy access to complete medical history and diagnostic test results of patients. This means there is less chance for error, fewer repeat tests, and better preventive care for the community.
