Southern NH Health Collaborates with Mass General Hospital for Kidney Transplant Evaluation Clinic

We are proud to announce a collaboration with Massachusetts General Hospital (MGH) to increase access to world-class kidney transplant care closer to home. Through this initiative, Southern New Hampshire Nephrology & Hypertension and MGH will begin to offer a kidney transplant evaluation clinic starting on June 17th in Nashua.

We understand the importance of extending nephrology services to patients who live in our community to make this essential care more accessible to patients who need it. We are pleased to expand our collaboration with MGH with this service,” said Southern New Hampshire Health Chief Medical Officer for Foundation Medical Partners Dr. Salvatore Vella.
 
MGH transplant teams travel to carefully chosen locations throughout New England to offer evaluations and consultations. The Kidney Transplant Program at MGH has proven success and performs more kidney transplants than any other institution in New England, and its patients enjoy some of the highest survival rates in the United States. Three years after surgery, more than 96% of MGH patients live with a functioning graft, compared to 90% nationwide.
 
With nationally trained and certified medical interpreters, the MGH team assists individuals and families who are Limited English Proficient or Deaf and Hard of Hearing. Additionally, qualifying candidates may receive resources and support through the Equity in Kidney Transplantation (EqKT) Initiative aimed at bridging access to transplant care for historically disadvantaged patients.
 
This program is open by self-referral or referral from providers from any hospital. Transplant candidates may be evaluated for a kidney transplant at Southern New Hampshire Nephrology & Hypertension by scheduling an appointment with the MGH team. To initiate the evaluation process, the Mass General Kidney Transplant Program requires a referral or appointment request.
